Grievance form: workplace concerns

Many concerns can be sorted out with a quick conversation, and that’s a good first try. When it can’t, this starts the formal process from the Employee Handbook. Each step gets a written response within 5 working days.

  1. Step 1: Your supervisor
  2. Step 2: Executive Director
  3. Step 3: CEO (final decision)

No retaliation. Demo doesn’t allow retaliation against anyone who raises a concern in good faith, files a grievance, or takes part in an investigation. If your concern is about your supervisor, or about discrimination, harassment, or retaliation, it skips your supervisor and goes straight to the Executive Director.

This will be investigated promptly. You also have the right to file with the U.S. Equal Employment Opportunity Commission at eeoc.gov.

It will go to the Executive Director instead, and your supervisor won’t be emailed about it.

Facts, in order: what happened, who was involved (by role), and how it affected your work. No client names. Describe them by role only, e.g. “an IOP client.”

Only Demo staff take part in the grievance process. Grievances are kept confidential and shared only with the people handling them.
