Forms & requests

Ask for something

Pick what you need. Each request goes to the person who handles it, and you get a copy by email.

Time off request Goes to your supervisor, who replies by email. Clock hours in Paylocity as usual. For example
  • A vacation day or a long weekend
  • A doctor’s appointment or a sick day
  • Jury duty, a funeral, or a family need
Supply request Goes to the Operations Manager. For example
  • Printer paper, ink, or pens
  • Group workbooks or handouts
  • Naloxone kits, first aid supplies, or drug test cups
IT help request Goes to whoever handles IT. For example
  • Reset a password, or you’re locked out of email or the EHR
  • Your laptop or iPad isn’t working
  • The camera or sound isn’t working for telehealth
  • An account for someone new
Training, CEU, or license submission Goes to your supervisor for approval, then onto your training record. For example
  • You finished CPR or naloxone training
  • You renewed your license or certification
  • You attended a CEU webinar
Emergency contact update Only leadership sees it. For example
  • You moved, or your contact’s number changed
  • You want to add a second contact
How-to guides Step-by-step guides for everyday tasks. For example
  • Review my pay stub
  • Reset my email password
  • Set up my email signature
Job referral Know someone who’d be great here? Tell us before they apply. For example
  • A former coworker who’s a great therapist
  • A friend in recovery who’d be a strong peer specialist
Question for Operations Common questions are answered right there. Anything else goes to Operations. For example
  • How much PTO do I have, and when can I use it?
  • My paycheck looks short
  • I need a letter proving I work here
  • When do my benefits start?

Need a workplace accommodation (disability, pregnancy, or religious)? Talk to your supervisor or any member of leadership. You can ask out loud; there’s no form to find first. For pay stubs, W-2s, or direct deposit, use Paylocity.
